The patrol entered a Kraal during a search for suspected insurgents while he remained on the Buffel Troop Carrier acting as the Gunner. An insurgent that was hiding in one of the huts fired an RPG-7 at the vehicle and the PG-7 Anti-Tank Rocket exploded at the position where he was standing, killing him instantly.

Killed in action, Ops Askari, ten members that were part of an Anti-Aircraft team deployed near Caiundu in Southern Angola were killed during a fierce fire-fight when their position was attacked by a numerically superior enemy force of more than 200 FAPLA troops. Sergeant Hendrik Albertus Oosthuyse volunteered to remain behind armed with a MAG machine-gun and due to his heroic actions, he delayed the enemy advance sufficiently to allow the remainder of the team to successfully evade capture.

- 1980 33002 — Lance Corporal Willem Hendrik Smit, 76320795BG, 2 South African Infantry Battalion (South African Infantry Corps), aged 20. Killed in action, while searching a Kraal in Northern Owamboland. The patrol entered a Kraal during a search for suspected insurgents while he remained on the Buffel Troop Carrier acting as the Gunner. An insurgent that was hiding in one of the huts fired an RPG-7 at the vehicle and the PG-7 Anti-Tank Rocket exploded at the position where he was standing, killing him instantly.

- 1989 32581 — Airman Gary Walter Pringle, 86336997PE, School of Logistical Training (South African Air Force), aged 19. Died in a Motor Vehicle Accident, in Edenvale area on the N3 highway near Modderfontein. Drove in the back of a truck. (3 deaths).

- 1990 31662 — Rifleman Tears Mashile, 88732193PB, 202 Battalion (1 Reconnaissance Regiment), aged 20. Killed in action, Ops Askari, ten members that were part of an Anti-Aircraft team deployed near Caiundu in Southern Angola were killed during a fierce fire-fight when their position was attacked by a numerically superior enemy force of more than 200 FAPLA troops. Sergeant Hendrik Albertus Oosthuyse volunteered to remain behind armed with a MAG machine-gun and due to his heroic actions, he delayed the enemy advance sufficiently to allow the remainder of the team to successfully evade capture.
