He, together with the B Squadron Commander and the Tech Advisor, were inspecting a broken down tank when a bomb fell near them, wounding Lt. Gray. A vehicle was fetched to transport him for medical attention but that too was was damaged by a bomb, killing Lt. Gray and wounding the B Squadron Commander, Major Buchanan.

Casualties were from heavy shelling by artillery and enemy tanks situated near Sartiano.
