The three-man Team was tracked and ambushed by a numerically superior force of SWAPO/PLAN insurgents and in the ensuing fire-fight, two Team members were killed and the third member, although wounded, successfully evaded capture until the helicopter arrived to extract the Team. Upon hearing the approaching helicopter, the SWAPO/PLAN force broke contact and withdrew from the area.

He together with others had boarded a suspect Iranian Freighter, the "Dhow Hoane" for a routine search and inspection operation and while he was disembarking from the ship, he slipped off the ladder and fell into the sea. Despite a 36-hour search, no trace of his body was ever found. He was no known grave and remains unaccounted for.

Killed in action, at Shona Mutamanjamba in Southern Angola while carrying out reconnaissance mission to locate a suspected SWAPO/PLAN Base in the area. The three-man Team was tracked and ambushed by a numerically superior force of SWAPO/PLAN insurgents and in the ensuing fire-fight, two Team members were killed and the third member, although wounded, successfully evaded capture until the helicopter arrived to extract the Team. Upon hearing the approaching helicopter, the SWAPO/PLAN force broke contact and withdrew from the area.

- 1998 31807 — Rifleman Matlhomola Sameul Molale, 94739547, 10 South African Infantry Battalion (South African Army), aged 23. Died of assault, assaulted by unknown parties while off duty, at Pongola.

- 2005 34298 — Director Malefetsane Louis Chali, 603896-4, Mpumalanga (South African Police Service), aged 33. Died in the execution of his duty as a member of the South African Police.

- 2008 30180 — Corporal Sibusiso Dube, United Nations Monuc Mission (SA Army), aged 30. Died after an explosion at the Nyiragongo base in Goma, DRC.

- 2009 42542 — Constable Seisa, 7091024-3, South African Police Service. Died as a result of their duties.

- 2012 39777 — Able Seaman Thulani Emmanual Mbuli, SAS Drakensberg (South African Navy), aged 19. Drowned, during anti-piracy operations in the Northern Mocambique Channel. He together with others had boarded a suspect Iranian Freighter, the "Dhow Hoane" for a routine search and inspection operation and while he was disembarking from the ship, he slipped off the ladder and fell into the sea. Despite a 36-hour search, no trace of his body was ever found. He was no known grave and remains unaccounted for.

- 2016 43061 — Warrant Officer Didi, 481440-1, South African Police Service. Died as a result of their duties.
