"25 Oct 1994: 02002781BE Lance Corporal Reginald Rapita from the South African Army (Member of the Bophuthatswana Defence Force) Died of unknown medical causes, at 1 Military Hospital. He apparently died as a result of taking some type of traditional medicine. He was 29."
21 Sep 2026: CauseDeath trimmed to the circumstance of death alone. On import it had been set to Mike Sandham's entire entry, repeating the date, service number, rank, name and unit that this record already holds in its own fields. His wording is quoted verbatim in the citation above, so nothing is lost. Defect found when Graham du Toit's Last Post of 21 Sep 2026 independently confirmed one of these records, Sikirila 49773.
