Lieutenant Sibusiso Madubela went on a murder rampage at 1 South African Infantry Battalion because of a salary problem, which he caused by himself for being Absent Without Leave (AWOL), and systematically started shooting serving members and civilian employees with his R4 Assault Rifle. His rampage came to an abrupt end when he was intercepted and shot dead by Lieutenant Anton Otto and Warrant Officer Class I Aldo Mattheus (who was also wounded in the fire-fight) while on his way back to the Unit Armoury to replenish his ammunition. Seven SANDF members and one female civilian employee were killed, and twelve SANDF members were wounded, one of them critically

Exact match otherwise. Stored Rank "Miss" left as-is (not a military rank, but Graham's own label "Civilian Worker (Miss)" is consistent).
Mike Sandham OMISSIONS: 16 SEPTEMBER (mailed 16 Sep 2026, credited The South Africa War Graves Project, southafricawargraves.org, search 17 Jan 1949 - 17 Feb 2017). Mike flags "the casualty not listed" among the eight killed in the 1 SAI shooting: "91725457CA Civilian Miss Marita Marlene Hamilton. She was 57", and asks whether this is a case for a civilian to be recognised. SHE IS ALREADY ON THE ROLL -- this record, added earlier and cross-checked against Graham du Toit's Last Post the same morning. "Not listed" refers to her absence from the daily post, not from the ROH. SAWGP's service number, forenames and age match this record exactly.
- Corps "South African Infantry Corps" (CorpsID 5175) CLEARED: a category error. She was a civilian employee at 1 South African Infantry Battalion, not a member of the Infantry Corps. The roll's own convention for civilians is Corps empty -- all 152 records with Rank "Civilian" carry no Corps. Regiment left as 1 SAI, which records where she worked and where she died.
- Unit2 "Army" cleared: junk from the known systemic Unit2 bug (UnitID2 was NULL).
- OPEN QUESTION for John, flagged not changed: Rank is "Miss", a courtesy title rather than a rank. The roll's dominant convention is Rank "Civilian" (152 records). Both sources here hedge the same way -- Graham writes "Civilian Worker (Miss)", SAWGP writes "Civilian Miss" -- so both treat "Civilian" as the category and "Miss" as the courtesy title.
