20194Second Lieutenant John Philpot, 253rd Coy. (Royal Engineers), aged 30. Died of gas poisoning. The poison gas had penetrated a mine in the trenches, and several of its occupants were overcome by it. Mr. Philpot insisted on going into the mine to take his part in trying to rescue the sufferers; but he was soon in the same condition as the others, and expired before the rescue party could reach him.

15896Lieutenant (E) William Joseph McCullagh, H.M.S. Mahratta (Royal Navy), aged 41. Killed in action, while escorting convoy JW-57 to Russia, was struck by two T5 Gnat torpedoes fired by U-990, off the coast of Norway, some 200 miles off Nordkapp. Only 16 of the 236 crew survived. A Catalina aircraft of 210 Squadron attacked and sank U-601 which was trailing the convoy. Although Mahratta was lost, the convoy was a success. It was the largest convoy ever sent to Russia.

29895Staff Sergeant Almerindo Mourao Da Costa, 75014688PE, 32 (Buffalo) Battalion (1 Reconnaissance Regiment), aged 35. Killed in action, officially reported missing during a Special Forces undercover operation at an undisclosed destination after his team walked into an enemy ambush while en-route to destroy a designated enemy target. It is, however, suspected that he actually died in a clandestine operation in Gwelo, Rhodesia, while attempting to use mines to blow up the Mambo Press offices. The mine detonated prematurely and there was nothing to recover from his body. He was a former member of the Portuguese FLECHAS, an elite unit of the DGS in Angola. He has no known grave and remains unaccounted for.

36380Sergeant Christiaan Petrus Momberg, W404222W, South West Africa Police Counter-Insurgency Wing: Ops-K Division (KOEVOET) (South West African Police), aged 23. Died of wounds, critically wounded in the head during a contact with SWAPO/PLAN insurgents in Northern Owamboland on 23 February 1987, during Counter-insurgency duty in South West Africa. Member of Koevoet. He was evacuated to Eenhana where he later lapsed into a coma prior to being evacuated to 1 Military Hospital in Pretoria. He remained on life support until declared brain dead on 25 February 1987.

30684Bombardier Clinton Hendricks, 84385459BG, 10 Anti Aircraft Regiment (South African Artillery), aged 19. Killed in action, contact with insurgents in South Eastern Angola, near Cuito, during Operation Hooper. He was operating with 61 Mechanized Battalion Group SAM-7 Anti-Aircraft team. Just as he was exiting the vehicle to engage Angolan Air Force MIG aircraft with a SAM-7, an enemy 82mm mortar bomb exploded next to his Ratel IFV, killing him instantly.
