A pilot for the Silver Falcons. During an airshow at Lanseria, the wing of his Impala separated. Although he successfully ejected, he was already very close to the ground and the downward vector caused his parachute and ejection seat to be sucked into the resulting fire-ball when the aircraft exploded after hitting the ground.

- 1991 49370 — Rifleman Kambinda, 31 Battalion. Died in service (internal conflict era, post-independence).

- 1993 34038 — Sergeant Barkhuyzen, 0405164-5, 17 Dist (Graaff Reinet) CE Div. Died in the execution of his duty as a member of the South African Police.

- 1993 32765 — Captain Charles Sergei Keith Rudnick, 85 Combat Flying School (South African Air Force (Corps)), aged 34. Died in Aircraft Accident, in Atlas MB326M Impala Mk I #489. A pilot for the Silver Falcons. During an airshow at Lanseria, the wing of his Impala separated. Although he successfully ejected, he was already very close to the ground and the downward vector caused his parachute and ejection seat to be sucked into the resulting fire-ball when the aircraft exploded after hitting the ground.

- 1995 37163 — Constable Pieterson, 0622092-4, Swapol-Tin Rundu. KOAS - COIN duty in Ovamboland.

- 1995 37190 — Sergeant Potelwa, 0514956-8, Eastern Cape. Died in Motor Vehicle Accident.

- 1996 36282 — Sergeant Maisha Phanuel Mohatle, 0630557-1, Gauteng, aged 23. Died in bus accident.

- 1996 37418 — Sergeant Scheffers, 0637679-7, South West Africa Police Counter-Insurgency Wing: Ops-K Division (KOEVOET). Died in Motor Vehicle Accident.

- 1997 31746 — Staff Sergeant Thulasizwe Cosmos Mbambo, 79890604PE, 121 South African Infantry Battalion, aged 46. Died in Military Vehicle Accident, on the N2 Highway at Mtubatuba. Vehicle caught alight and he burnt to death.
