"14 Dec 1989: 86635539BG Signaller Jacob Ferdinand Leonardus Kerkhoff from 5 Signal Regiment, South African Corps of Signals Died in Private Vehicle Accident, on the Ben Schoeman Highway, between Pretoria and Johannesburg. Vehicle left the road, rolled and struck trees. 3 passengers were injured. He was off duty. He was 19."

On import it had been set to Mike Sandham's entire entry, repeating the date, service number, rank, name and unit that this record already holds in its own fields. His wording is quoted verbatim in the citation above, so nothing is lost. Defect found when Graham du Toit's Last Post of 21 Sep 2026 independently confirmed one of these records, Sikirila 49773.
